This certificate shows that the Soldat has been examined at the Entlausunsanstalt (De-lousing Station) and he is free of visible

disease and vermin.  This form was to be carried in the Soldbuch and produced on demand. 

It is interesting that this is unused, but has been pre-signed and stamped.
